    Comparison of Sexual Functions in Transobturator Tape Surgeries Performed with Inside-Out Technique and Outside-In Technique

    Objectives: To compare the postoperative sexual function results of inside-out transobturator tape (TOT) and outside-in TOT in the treatment of stress urinary incontinence. Materials and Methods: The study included 54 women with stress incontinence between 2020 and 2022. Group 1 patients underwent the inside-out transobturator tape (TOT) procedure, while Group 2 patients underwent the outside-in TOT procedure. Female Sexual Function Index (FSFI), Female Sexual Distress Scale-R (FSDS-R), Golombok-Rust Sexual Satisfaction Scale, Quality of Sexual Experience Scale (QSES), and Patient Global Impression of Improvement (PGI-I) were applied to the patients preoperatively and 18 months postoperatively to compare the results of the 2 surgical procedures on sexual functions. Results: Postoperative results were statistically significantly higher in both groups. Group 1 was found to be more effective for FSDS, FSFI sexual desire, FSFI sexual arousal, FSFI lubrication, FSFI orgasm, FSFI pain, and FSFI total variables. When the p values of Golombok levels in Groups 1 and 2 were examined, it was concluded that there was a significant difference between the groups because the p values of communication, avoidance, satisfaction, and vaginusmus scales were <0.05. Conclusion: When the methods were compared with each other, it was observed that the increase in sexual functions was more significant in the group with inside-out TOT procedure.

    Does the Surgical Margin Affect Sexual Function Following Linear Labiaplasty Technique?

    Background How much labial tissue should be left after labiaplasty is a controversial issue.Objectives The objective was to investigate the effect of residual labial tissue following labiaplasty operations on sexual function.Methods A total of 150 females who underwent labiaplasty between 2019 and 2021 and their partners were included in the study. In this retrospective study, linear labiaplasty technique was applied according to the patient's request. Patients were either below 1 cm or above 1 cm according to the remaining labial width at follow-up. The remaining labial tissues were classified according to the Motakef classification. Patients with a labial width above 1 cm were included in Group 1, and patients with a labial width below 1 cm were included in Group 2. Sexual function, sexual experience, body image, and erectile function of their partners were compared preoperatively and 2 years postoperatively with appropriate questionnaires.Results Sexual function, sexual experience, and body image of females improved in all patients. However, the improvement was more apparent with with remaining labial tissue of greater than 1 cm. The increase in the survey results of the partners was found to be similar.Conclusions The sexual function of the patients was better when the width of the labium was longer than 1 cm. This may be due to better protection of the neurovascular structures. This favorable effect was also seen in the subjective evaluation of the partners.

    Partner Satisfaction in Labiaplasty Patients

    Background Labiaplasty, which has become increasingly popular in recent years, is chosen by women for both cosmetic and functional reasons. It creates significant changes in female sexuality, resulting in high satisfaction rates, but there are limited data on its effects on male sexual response.Objectives The aim of this study was to investigate the effects of labiaplasty on partners.Methods The partners of 49 patients who underwent labiaplasty between January 2020 and May 2023 were included in the study. Male Sexual Health Questionnaire Ejaculatory Dysfunction (MSHQ-EjD), Golombok-Rust Sexual Satisfaction Scale, and New Sexual Satisfaction Scale questionnaires were administered to the partners preoperatively and 6 months postoperatively.Results There was no significant difference between preoperative and postoperative responses to the MSHQ-EjD questionnaire. The postoperative increase in the New Sexual Satisfaction Scale compared with the preoperative score was statistically significant. Statistically significant positive changes were observed postoperatively in the Golombok-Rust Sexual Satisfaction Scale categories of intercourse frequency, communication, satisfaction, and nature of sexual intercourse.Conclusions Labiaplasty operation had positive effects on male sexual response but had no effect on ejaculation function and difficulty.

    The Effect of Tension-Free Vaginal Tape Surgery on Mixed Urinary Incontinence Symptoms and Anatomical Parameters: The Role of Transperineal Ultrasonography in Follow-Up

    Objective: Tension-free vaginal tape (TVT) in patients with mixed urinary incontinence (MUI) operation was aimed to show the changes in stress and urge incontinence symptoms and quality of life.Materials and Methods: This prospective study included 36 patients with MUI who underwent TVT operation. Bladder neck descent, retrovesical angle, pubourethral distance, urethral thickness, detrusor thickness, bladder, rectum, and uterus descent, and urethral rotation angle were evaluated preoperatively and 2 years postoperatively by transperineal ultrasonography, and Pelvic Organ Prolapse Quantification staging was performed. Preoperative and postoperative international consultation on incontinence questionnaire short form (ICIQ-SF) and overactive bladder questionnaire (OAB-V8) questionnaires were also performed.Results: The mean ICIQ-SF questionnaire score decreased from 16.39 preoperatively to 5.28 postoperatively. Urge symptoms assessed by the OAB-V8 questionnaire improved compared with the preoperative period (mean scores were 29.22 and 9, respectively). Nocturia mean score decreased from 2.31 to 0.81, while nocturnal voiding rates improved significantly (mean scores were 3.39 and 1.14, respectively). It was statistically proven that the effects of urinary incontinence on overall quality of life decreased after TVT (mean scores were 7.72 and 2.57, respectively). Furthermore, the mean score of bladder neck descent was 33.27 +/- 3.90 preoperatively and 17.19 +/- 3.91 postoperatively; the mean score of pubourethral distance was 12.11 +/- 1.95 preoperatively and 7.83 +/- 1.32 postoperatively; the mean score of urethral thickness was 8.69 +/- 1.58 preoperatively and 5.08 +/- 1.05 postoperatively; the mean score of detrusor thickness was 7.28 +/- 1.52 preoperatively and 4.92 +/- 1.02 postoperatively. A statistically significant difference was found between the mean scores of these parameters. In addition, retrovesical angle and urethral rotation angles decreased below the cut-off value postoperatively.Conclusions: In patients who underwent TVT for stress incontinence, if voiding functions can be restored to near normal, urge and stress symptoms improve.
