Öğe Anatomical surgical approach to lateral ventricle masses and histopathological findings of the brain(Via Medica, 2024) Ünlü, İlker; Alptekin, Mehmet; Tuncer, Mehmet CudiBackground: The selection of surgical approach to the lateral ventricular masses includes difficulties due to their deep localisations and close proximity to the vascular and the eloquent brain structures. The most appropriate approach that should be chosen in surgical treatment of lateral ventricular masses is still controversial. In this study, the factors in the choice of surgical approach to the lateral ventricle masses and the results of them were investigated. Materials and methods: In this study, 80 patients who underwent surgery in our clinic due to the lateral ventricular masses were retrospectively analysed between the years 2002 and 2013. All the cases were evaluated in terms of clinical and neuroradiological results pre- and postoperatively. In 24 cases the anterior interhemispheric transcallosal, in 4 cases the posterior interhemispheric transcallosal, in 30 cases the posterior interhemispheric precuneal, in 14 cases the transcortical, and in 8 cases the combined surgical approaches were performed. Results: Gender distribution of the cases were 45 male/35 female, and the mean age of them was 31.7 years (7 month — 73 years). In 64 patients the gross total resection was performed, whereas in 16 patients subtotal resection was performed due to the infiltration of eloquent brain areas. In the histopathological examination 52 neuroepithelial, 8 mixed neuroglial, 3 meningeal, 2 lympho-hematopoietic system, 1 pine blastoma, 1 germ cell, 5 metastatic, and 8 other benign masses were observed. After surgery, additional neurological deficits developed in 9 patients. Mortality was observed in 6 patients postoperatively. The average follow-up time was 13 (1–83) months. Conclusions: The essential factors that affect the results of surgical treatment of lateral ventricular masses are the size of the mass, histopathology, location, extension, and the relationship with the neurovascular structures. The goal of surgery is to provide a histopathological diagnosis, gross total resection, if possible, and to normalise the flow of cerebrospinal fluid by eliminating the mass effect of pressure. © 2024 Via Medica.Öğe Effects of Blood Hemoglobin Levels on Vertebral Discal Degeneration(2023) Cetin, Eyup; Şah, Volkan; Arslan, Mustafa; Arabacı, Ozkan; Akyol, Mehmet Edip; Ünlü, İlkerIntroduction: Vertebral discopathies are one of the most important causes of low back pain, which is very common in the community. Although there are many biochemical and mechanical causes of discogenic low back pain, the most important is discal degeneration. Many studies have been conducted suggesting that disc degeneration could be associated with various factors. In our study, we aimed to reveal a possible relationship between disc degeneration and levels of hemoglobin (HGB). Methods: We investigated the connection between discal degeneration and certain parameters, especially HGB levels, in a total of 174 patients in two separate groups; those with and without disc degeneration. Age, weight, height, body mass index, chronic diseases, mean HGB, hematocrit (HTC) level, white blood cell (WBC), high-density lipoprotein, low-density lipoprotein (LDL), and triglyceride (TG) values of the patients were recorded. Results: No significant difference was detected (p>0.05) between the groups with and without degeneration in terms of HGB, HTC, WBC, and LDL values. The TG level in the group with disc degeneration was significantly (p<0.05) higher than the group without disc degeneration. Discussion and Conclusion: Lumbar disc degeneration is a public health problem that continues to be discussed in the literature. There was no significant difference in terms of HGB levels between the groups, with and without degenerated disc.Öğe Honokiol Showed Neuroprotective Effects on Cerebellum in Spinal Cord Injury in Rats(Colegio de Farmaceuticos de la Provincia de Buenos Aires, 2024) Ünlü, İlker; Tutal Gürsoy, Görkem; Deveci, EnginSpinal cord injury (SCI) is developed by primarily traumatic external causes or secondarily hemorrhage and edema in cord. SCI causes neurological and multiorgan dysfunction. In the present study, effects of honokiol was investigated on cerebellar tissue in experimental SCI model by biochemical and histochemical analysis. Thirty-male Sprague Dawley rats were assigned to three groups. SCI was developed by dropping a steel rod with a diameter of 3 mm and a weight of 10 g was dropped from 10 cm to the spinal cord T7-9 vertebras under anesthesia. Upon SCI, 50 mg/kg honokiol was intraperitoneally introduced to animals for postoperative 7 days. Cerebellar tissue samples were fixed in zinc-formalin and processed for further histochemical analysis. Blood samples were collected for malondialdehyde (MDA), total antioxidant status (TAS) and oxidant status (TOS). Results were statistically analyzed and recorded. MDA and TOS values were higher and TAS values were lower in SCI compared to sham group. Honokiol restored the values close to values of sham group. Histopathological scores were increased in SCI group but honokiol treatment alleviated the pathologies and restored scores in SCI+honokiol group. IL-6 and Bax expression was mainly negative in sham groups. Glial cells, Purkinje cells and granular cells of cerebellum showed negative IL-6 and Bax immune reaction. SCI increased the expression of IL-6 and Bax in basket cells, Purkinje cells, glia cells of the substantia alba, in ganglions layer. After SCI, honokiol administration reduced the immune reaction of IL-6 and Bax in cerebellar cells and substansia alba. SCI caused hypoxia and tissue degeneration and inflammation in cerebellum. Honokiol administration may prevent cell degeneration and apoptosis by balancing cytokine activity to prevent oxidative stress and may be an alternative in cerebellar therapy. © 2024, Colegio de Farmaceuticos de la Provincia de Buenos Aires.
